Release and Permission Forms
Teens and Parents of Teens! Please fill out these release forms and send them via email to lifechanger@lifechangerchurch.com, or drop them by the Church Office Tuesday through Thursday 9:00 Am - 1:00 Pm, they can also be given directly to Pastors David and Sherri Estes. These need to be completed BEFORE we can drive anyone anywhere so please get them done ASAP!!!!!!
*Note: Not all activities listed in the permission form will be participated in or offered at camps, lock-in, or other events.*
Parents! Please fill out these release forms

These need completed in the event that we have a children's church activity day, VBS, or similar event for the kids.
